New Client Intake Form

Once your New Client Intake Form is submitted, staff at the Law Office of Dean A. Campbell, P.A. will review your matter to ensure it is within an established area of practice. You will be e-mailed a response. You may be asked for additional information including, but not limited to, the name(s) of the opposing party so that a conflict of interest check can be conducted.

Please be aware that submission of this New Client Intake Form does not establish an attorney-client relationship between you and the attorney. The attorney will not take any action on your behalf until after an initial consultation is held.
